Hospital General Castañer earns 4th NCQA recognition
Hospital General Castañer, along with its polyclinics in Adjuntas and Jayuya, has been recognized for the fourth consecutive year by the National Committee for Quality Assurance (NCQA) for meeting patient-centered care standards.
The hospital, which operates as a Health Center 330, continues to meet the NCQA’s requirements under its Patient-Centered Medical Home (PCMH) program.
“This achievement validates the high standards of quality, access, prevention and collaboration that characterize Hospital General Castañer’s work and its network of community clinics,” said Giovanni Ortiz, CEO of Hospital General Castañer. “With this recognition, we reaffirm our mission to offer patient-centered health services accessible to mountain communities.”
The PCMH model emphasizes long-term relationships between providers and patients, encouraging consistent engagement and individualized care rather than one-time visits. It also promotes shared decision-making, preventive care and patient education to help individuals manage their own health more effectively.
To earn the NCQA distinction, the hospital demonstrated adherence to PCMH principles, including expanded patient access, use of health information technology and coordinated care across its network.
